小朋友食益生菌有用？睇研究要睇「菌株」

家長常問：益生菌，邊隻先啱？

超市同藥房嘅益生菌五花八門，家長難免諗：畀小朋友食，有冇用？今日睇兩份公開文獻點講。本文只整理研究發現，唔係叫家長自行購買任何產品。

歐洲兒童腸胃營養學會 2023：要睇「菌株」

Szajewska 等（2023）喺《J Pediatr Gastroenterol Nutr》發表立場文件，檢視至 2021 年 12 月嘅研究，並規定同一菌株最少有兩項隨機對照試驗，先會作出建議。文件寫：「The effects of probiotics are considered to be strain specific.」即係要按特定菌株去睇，而唔係「益生菌」一概而論。

文件亦寫：「The use of probiotics with no documented health benefits should be discouraged.」多數建議屬「弱建議」，證據確定性由 “very low” 至 “moderate”。就兒童功能性便秘，文件話已研究嘅益生菌 “due to the lack of efficacy” 而不建議。

2025 傘式統合分析：證據未有定論

Chen 等（2025）喺《BMC Pediatr》整合 35 項有關兒童腹瀉、便秘嘅系統性回顧及統合分析（檢索至 2025 年 5 月）。作者將證據確定性評為低。作者亦寫：「The current evidence regarding the impact of probiotics on diarrhea and constipation outcomes in children is inconclusive。」

不同研究結果唔一致

便秘方面，ESPGHAN 立場文件唔建議用益生菌，Chen 等就冇下同樣結論，只話現有證據未有定論。Chen 等指多數分析異質性極高，可能同菌株、劑量、療程同參與者特徵不同有關。結果定義不一，亦係限制之一。

其他限制

- ESPGHAN 工作小組冇檢視益生菌安全性；多位作者披露同業界有資助或講者關係。

- 建議只針對指定菌株，唔等於市面上類似產品同樣適用。

- Chen 等屬傘式分析，有重複計算同一批試驗嘅風險。

香港背景

衞生署意見（經消費者委員會《選擇》月刊轉載，屬二手資料，刊出日期未核實）：「嬰幼兒及孕婦選擇服用益生菌補充劑前，必須諮詢醫生意見。」
